Based off-site at the Local, the Towards Change Lead will be employed by Mildura Base Public Hospital and will provide leadership and support to the Towards Change Team. The Towards Change team is a multi disciplinary team of mental health clinicians offering evidence based therapeutic interventions alongside our lived experience colleagues in the Support and Connect team.

The role will work closely with the Clinical Manager and the Local’s leadership team, to help to foster a culture of collaborative relationships amongst all disciplines working within The Local.

The HOPE Program has been developed to provide assertive integrated community-based aftercare for consumers experiencing a suicidal crisis.  Under the general direction of Clinicians, Wellbeing Support Workers will work collaboratively with consumers and their carers to address their identified areas of psychosocial need.  This will include liaising on behalf of, and assisting consumers to access a broad range of community supports; human services; Government agencies; and consult with existing supports.
